Most of us have probably heard the old acronym RICE 🍚when it comes to healing an acute injury. Rest, ice, compression, elevation. I don’t know that there’s enough research to say this isn’t effective in treating acute injury, but I will say that it’s certainly not the best way to optimize healing after said acute injury!  RICE is intended to reduce inflammation which can be beneficial in treating certain pathologies, but, by the same token, increased blood flow (or the inflammatory cascade) is also how our body heals. Are we trying to get in the way of that?

So...for all you RICE 🍚lovers out there, tell me; have you ever tried MEAT? 🥩🍗🍖Movement, exercise, analgesia and treatment. MEAT increases blood flow to injured areas in order to optimize the healing process. Soft tissue structures like ligaments, tendons, and cartilage don’t get a lot of blood supply to begin with, so reducing blood flow with RICE may actually prolong healing.

Há praticamente um ano atrás depois de um jogo pelo Campeonato Paulista de Futebol Americano comecei a sentir fortes dores no joelho, mal conseguia andar 500m sem sentir uma dor extrema. Após o diagnóstico da Condromalácia Patelar médicos disseram que eu provavelmente nunca mais ia conseguir correr do mesmo jeito que eu corria e que deveria começar a fazer injeções de ácido no joelho.
Um mês após o diagnóstico uma pessoa mudou totalmente minha rotina e deu uma vida nova as minhas dificuldades @gonsachileno e a cada dia que passava o corpo dava resultado.
Hj após 1 ano estou praticamente sem dores e melhor condicionado.

Time for a formal update. Good news: the weight loss is going slow and steady, I'm down nearly 10lbs from the beginning of the month. Bad news: even after a 2 week deload, my back still can't handle heavy deadlifts. I nearly collapsed lifting 315lbs (don't forget, my max is, or was, 500lbs). Even after an x-ray I still have my suspicions that it may be a pinched nerve, or (god forbid) a slipped/bulging disc. But for now, all I can do is train everything else twice as hard to compensate. Keep the faith brothers 🙏

To me, this is a quote I always think of when planning my surgeries. It’s critical that we have a well thought out plan of action, especially in orthopedics where preoperative templating can make or break a case. At the same time, even the best laid plans go awry. With that being said, here are some critical ‘planning’ KEYS🗝 that I find valuable:

1️⃣Visualization: For many cases I’ll close my eyes and visualize every single step from start to finish. This includes OR setup, positioning and then surgical execution through dressing placement. I’ve found that when I do this preoperatively I am not only more ‘successful’ in my plan of action but more efficient and relaxed during surgery.

2️⃣Anticipation: Along each step I’ll anticipate the possibilities that may occur including any potential complications or key anatomy that I need to be cognizant of.
🛠
3️⃣A-Z: I’ll have my primary plan but I make sure I consider every possible scenario and how I would ‘bail out’. This not only includes surgical techniques but also what equipment or hardware I may need for each scenario. (don’t forget to request it on your OR request sheets!!!)

4️⃣Surgical Algorithm: Similarly (but different!) I’ll have a comprehensive algorithm for each scenario. If ‘a’ happens do ‘b’ but if ‘c’ happens do ‘d’… Essentially I’m trying to consider every route of success and how I’d get there

5️⃣Rely on others: I’ll often use as many resources to consider every viable option for success. This includes comprehensive literature reviews, case discussions with peers and mentors, and especially reviewing old cases to refresh on what worked and what didn’t.

To me, this is a quote I always think of when planning my surgeries. It’s critical that we have a well thought out plan of action, especially in orthopedics where preoperative templating can make or break a case. At the same time, even the best laid plans go awry. With that being said, here are some critical ‘planning’ KEYS🗝 that I find valuable:

1️⃣Visualization: For many cases I’ll close my eyes and visualize every single step from start to finish. This includes OR setup, positioning and then surgical execution through dressing placement. I’ve found that when I do this preoperatively I am not only more ‘successful’ in my plan of action but more efficient and relaxed during surgery.

2️⃣Anticipation: Along each step I’ll anticipate the possibilities that may occur including any potential complications or key anatomy that I need to be cognizant of.

3️⃣A-Z: I’ll have my primary plan but I make sure I consider every possible scenario and how I would ‘bail out’. This not only includes surgical techniques but also what equipment or hardware I may need for each scenario. (don’t forget to request it on your OR request sheets!!!)

4️⃣Surgical Algorithm: Similarly (but different!) I’ll have a comprehensive algorithm for each scenario. If ‘a’ happens do ‘b’ but if ‘c’ happens do ‘d’… Essentially I’m trying to consider every route of success and how I’d get there

5️⃣Rely on others: I’ll often use as many resources to consider every viable option for success. This includes comprehensive literature reviews, case discussions with peers and mentors, and especially reviewing old cases to refresh on what worked and what didn’t.

It can be easy to miss the big picture when comparing ourselves to others on social media or focusing too much on the little details. However, whether it’s in the rehab or training setting, I think it’s important from time to time to take a step back to reflect on the growth that has occurred over a given period. The following examples are just a small number of ways to do just that, but you do not have to limit yourself to these as some may not apply to you.

📌Can you lift HEAVIER? This is the goal that is probably sought after most often, but you’ll see that it’s not the only way to demonstrate progress.

📌Can you do more REPS? 11 reps instead of 10 at the same weight is a 10% improvement. That’s huge in my opinion.

📌Can you add RANGE? Turning a partial squat into a parallel squat is no trivial task.

📌Do you LOOK better? Whatever that means to you. Losing fat, gaining muscle, or maybe this category just doesn’t apply to your goals.

📌Do you FEEL better? Has your mood improved? Is your sleep better? Is your memory enhanced? Exercise does some amazing things to the body.

📌Do you have LESS pain? Maybe you’re not quite back to running, but those stairs don’t bother you like they used to.

📌Are you LEARNING? We learn from failure. If you missed a lift or hurt yourself from pushing too hard, there’s a lesson to be had there. It’s a small drop in the bucket and still adds to the progress in the long run.

📌Are you HEALTHIER? Blood pressure improved? Medication dosage decreased? Fantastic!

📌Are you CONSISTENT? Sometimes the hardest thing to do is show up. There’s nothing wrong with setting a goal of getting in just 15 minutes of exercise every day. Other goals will fall in line when you’re more consistent.

Not an exciting training video but a specific one. Had a lot of issues with scapula/sholder stability for a while now. 
Pull ups were previously painful but getting there again. Notice how at the top of the movement I struggle with depressing the scapula in the first video. 
2nd video is an exercise I despise, however i find myself cheating by lifting my scapula up. Me and @trapsliketodd put in a physical cue by banding the position down to eliminate the cheat... holy shit thats hard 😂😂 Getting back there tho

Among the established risk factors for injury in runners, the biggest predictor of future injury is a history of prior injury (Van der Worp 2015, Saragiotto 2014). Since this is a non-modifiable risk factor, it does not give a clinician, coach, or athlete a target to reduce the risk of injury. Common treatments for running-related injuries include stretching, gait retraining, early mobilization, bracing, orthoses, gradual return to activity, eccentric exercises, heavy slow resistance training, and a host of non-efficacious modalities that one can explore on social media (Arnold 2018, Beyer 2015, Instagram). While none of these have been prospectively studied in endurance sports, we actually do have evidence on stretching, eccentric exercise, and heavy slow resistance training with respect to injury risk reduction among runners.

The familiar Barbell Medicine reader is saying, "Wait... did they just say stretching?" Indeed: the most recent Cochrane Review on interventions for running-related injuries from 2011 showed that stretching does not reduce the risk of injuries (Yeung 2011). We also have decent evidence from Laureson et al. 2014 that stretching is not an effective way of reducing the risk of injury. This same paper did go on to say the resistance training reduced all injuries by one-third and reduced overuse injuries by half - and that kind of statement deserves elaboration given the traditional dogma on resistance training among the endurance population.
